Ways to File Your Claim

Phone: Call us at 1-800-375-2075 anytime day or night to report your claim.  Have your policy number ready when you call, and be prepared to give us details about what happened.

Online: Use our online forms to report your claim.  The information you provide online is encrypted by Verisign, so anything you enter is protected.  To access the appropriate online claims form, click on the product you have insured with us:

Fax or Mail:  You can fax your claim to us at 513-947-4040.  Or, you can mail it to:  American Modern Insurance Group, Attn:  Claims Processing, PO Box 5323, Cincinnati Ohio 45201-5323.  When sending your claim via fax or mail, please remember to include the following information:

• Policy number
• Policyholder’s full name and mailing address
• The name and phone number of someone we can contact throughout the entire claims process
• A brief description of the loss, including:
  • What was damaged/lost
  • When did the loss occur (date and specific time of day)
  • Note any personal injuries that also occurred
  • Whether you believe you need to make temporary repairs
  • Whether an ambulance was called, or fire or police departments were notified
